Admissions Process

CCHS requires the following items as part of the admission process:

1. Birth Certificate

A copy of the child’s birth certificate must be in the school records.
 
2. Immunization Records
Immunizations must be up-to-date according to North Carolina law. A physician, health clinic, or local health department administering required vaccines must give a Certificate of Immunization to the person who presented the child for immunization. Immunization requirements can be found at the following website: http://www.immunizenc.com/. The certificate/record must include the following information:
  • Name, sex, and date of birth
  • Name and address of parent or guardian
  • Number of vaccine doses given
  • Date vaccines were given (month and year is acceptable for out-of-state transfer students)
  • Signature of physician or health department stamp
3. Academic and Discipline Records
Parents are required to provide a current report card and achievement test scores (i.e. Stanford Achievement Test Battery, California Achievement Test, PSAT, or SAT). In addition to academic records, CCHS also requires any discipline records from previous schools and from any learning institution prior to a home school situation.
 
4. Recommendation Letters
 Prospective students are to provide a reference letter regarding their character, academic abilities, and community involvement from a local pastor or youth pastor, a teacher or principal, and a non-family member (i.e. employer, neighbor, mentor, etc.).
